A lot of people are having this kind of issue today. Open Plex Server and check if you're doing remote streaming with transcoding. For some reason, a lot of people are reporting remote access is not working, and Plex is using the Relay feature to stream video content via their infrastructure. Which limits playback to 2Mbit and 720p.

On some of my 4K content, even though the Plex Web app confirms it is using Direct Play, I am still getting buffering spots during playback telling me the connection to the server is not fast enough to play this video.As mentioned above, direct play is one of the methods Plex uses to stream media to a streaming client. Out of the three methods, direct play uses the least amount of resources on the server. The reason for the low server resource usage is because when a file is direct played, it is sent to the device without being changed. When playing titles with Direct Play it works great on bandwidths up to 80-90mbps (4K). When playing with Direct Stream (Transcode MKV -> MPEGTS, with SRT subtitles), even though the Transcode is throttled and the server is capable of 40-50x speed transcoding, it keeps buffering every 10 seconds. The server is remote but videos play fine on my phone with the plex app and on my windows machine with the plex app. All the devices on the same WiFi and all doing Direct play.
